UnitedHealth Group raised its full-year profit forecast after third-quarter earnings exceeded Wall Street expectations, providing further evidence that the healthcare giant’s efforts to control medical costs and rebuild margins are gaining traction.

UnitedHealth now expects adjusted 2026 earnings of at least $20.50 per share, up from its previous forecast of $19.50 to $20. Analysts had expected approximately $19.86.

Adjusted third-quarter earnings reached $4.69 per share, compared with $2.92 a year earlier and an analyst consensus of about $4.14.

Revenue totaled approximately $112.7 billion, while earnings from operations reached $6.6 billion.

One of the most closely watched figures was the company’s medical-cost ratio, which measures the percentage of insurance premiums spent on patient care.

The ratio improved to 88.4%.

UnitedHealth has spent much of the past two years attempting to stabilize costs following unusually heavy medical utilization, operational disruptions and a major cyberattack.

The latest results suggest that effort is producing measurable improvement.

Operating income increased to approximately $6.6 billion, supported by better performance at Optum Health and improved economics in the company’s insurance operations.

UnitedHealthcare revenue slipped slightly to about $86.3 billion, while membership declined in several categories, including employer-sponsored, Medicaid and Medicare Advantage plans.

That makes the improvement in profitability particularly significant: the company is producing stronger earnings even while parts of its membership base contract.

UnitedHealth also benefited from approximately $610 million of favorable reserve development during the quarter.

Investors responded positively, with the shares rising in premarket trading following the announcement.

The company has now raised its 2026 profit outlook three times, shifting the market’s attention increasingly toward 2027.

The next major test will be whether lower medical-cost pressure proves durable and whether Optum can continue improving margins without sacrificing growth.
